4. Birth registration form (download from the website or obtain at the Embassy).
5. Original copies of parents’ passports
6.  Original and copies of parents’ National IDs
7. Copy of child’s Omani Certificate of acceptance of birth report – (certified and legalized by the Ministry of Foreign Affairs)
8. Authentication fee – OMR 5.000.
9. Processing fee – OMR. 10.000.

• Statement explaining loss of original.

• Duplicate birth certificate issued to foreigners born in Kenya.
• Written statement explaining when and the place of birth in Kenya as well as the circumstances under which it got lost.
• Birth notification slip issued by hospital (for original), or photocopy of original birth certificate (for duplicate).

• Statement explaining reasons for application.
